Problem for some stbs with channel Heritage 4K is format 3840x2160 / H264 (not H265).

Maybe related to it reporting the resolution as 3840x2160 ?

My guess is that some STB's don't have hardware acceleration for decoding 4K h.264 but only for 4K h.265.
Meaning that they're left with software decoding and that's something that CPU has to do.
Probably some boxes don't have strong enough CPU for that operation.
And you're left with no picture at all or with some distorted picture.
Also I can confirm that channel is 3840x2160. Tested with my PC card.
