Fixed major bug in cooldown ratelimit functionality.
In cooldownstate = 2, check went wrong to transit to cooldownstate 0 or 1.
In the transition to 0, was just an optic bug.
The transition to cooldownstate 1, some important ecmrl structs got lost. Now the last reader->ratelimitecm
ecm request are kept. These channels are being handled.
Further, at the moment of the discovery that cooldown delay time cooldown[0] is elapsed. There is checked how
many ecm request were active at the exact moment, when cooldown delay time was elapsed (i.e. cooldowntime + cooldown[0]).

csctapi/azbox: Fix azbox activate function.
In r7872 I have mistakenly added skip_extra_atr_parsing parsing
flag to azbox card reader. This flag is wrong for this kind of reader
and probably causes card activation errors in this case.

Removed the caid / betatunnel check on cacheanswers.
Since the cacheanswer is only valid for a very short time I think
for cacheanswers we can rely on csp-hash and MD5 hash checksum of the ecm.
If they match the ecm must be the same even if caid is different.
