EnF settings

Updates (mainly for E2v4 lists):
- added FTA bouquets to each satellite
- removed and re-grouped some category bouquets
(olny 7 categories now from 10 before: "HDTV", "FTA", "Movies", "Sport", "Music", "Documentaries", "Erotic")
- imposed a limit of at least 10 channels to create a bouquet for a provider
(providers with less than 10 channels are still listed under "Other providers")
- added extra category markers for bouquets like "HDTV" and "FTA"
(only if a provider has at least 10 channels)
- changed/split the marker "Other" (provider or category) to "Unknown" (provider) and "Other" (category)
