- Fix for current DISH MECM.

- Rewrote support for file PMT's (for DGStation boxes).
This should fix rqcamd's problems on Mutant 200's.
I tested this on 200's and 400's and appears to be working well.
Please report if you see any problems.

- Added port for IPBOX 9000HD and CubeRevo HD. Both are high definition Linux boxes
based off STmicro's ST40 processor (don't forget to set box_type=3).

NOTE: Other ST40 boxes with Engima 2 *should* work, like the Kathrein UFS910 HD and
Qbox HD, i have not tested it on those however, if you have the means, try it and
report your results.
